To include “Failures by Mission Phase” use the checkbox on the Failure Effects Options Panel Showing Mission Phases in FMECA , where multiple instances of a failure effect have separate causes, and the effects, severities and attribute Test limits are associated with different mission phase(s).
When the FMECA is generated the Mission Phases will show up as shown below.
With this approach, the applicable Mission Phases for each effect are listed in a separate Mission Phases line item entry in the FMECA. Note here that the Severity Class varies for different Mission Phases.
