With the Context Panel showing the Failure Effect, Select the Failure Effect to add Cause as shown above.

Use right hand Button on Combined Effect and select Add Causes to open window shown below "Select Failure Effect Causes"

Use Filter box to reduce number of causes in window as shown below.

Select Cause and then select OK
The dialog has a text filter box (so that you can display only effects that include specified text) as well as a dropbox that lets you filter the displayed list by effect type. The options are: All Effect Types, Object Effects, Intermediate Effects, and External Events.
When the correct Effect Cause is selected the OK box at the bottom will turn from gray to black so the cause can be selected
